Here in Arizona, we live in an area where people get a higher than normal amount of sun exposure. And there is no doubt about it; the sun can damage our skin. There are two things to consider, one is damaging to your health and the other will cause you to appear to age faster. Depending on your genetics, your exposure to the sun and your skin type, skin cancers (squamous cell carcinoma, basal cell carcinoma, melanoma) are a real danger.

The other side of sun exposure is that it can damage your skin and make you look much older and more weathered than you may be. Skin is damaged with too much sun exposure. You can save yourself a lot of effort and money in the future if as a young person you protect yourself now. We recommend these specific instructions:
You must reapply every three hours. No matter how “waterproof” it is, after three hours, you will lose coverage and protection.
Use a 15 SPF or higher with a metal ion ingredient like zinc oxide. This will protect you from UVA and UVB rays.

Keep in mind that not everyone can get a sun tan but everyone can get sun damage. The Fitzpatrick Scale was developed in 1975 by Thomas B. Fitzpatrick. It is a way to classify the response of different types of skin to UV light. Fair complected, light eyes is a Fitzpatrick 1 and the other end progresses in darkness to be classified as a Ftizpatrick 7. A Fitzpatrick 1 will never get a tan and will always burn and then the spectrum continues until you get to a Fitzpatrick 7 which describes the complexion of someone who will never burn and will always be dark when exposed to sun. Because a Fitzpatrick 1 has less melatonin, they are at greater risk for skin cancers and need to protect themselves and accept that a tan is not going to happen for them. Melatonin gives a certain amount of natural sun protection, so the less melatonin you have, the less natural protection you have.

From an over all health standpoint, regardless of your known risk factors, it makes sense to protect yourself from too much sun exposure at all times. Some of the damage may already have been done in your childhood, but it’s never too late to start taking care of your skin and your health. The availability to sunblocks that are easy to use and highly effective are many. I will recommend sun block to every single patient I see regardless of the reason for their visit.

With today’s news about (former French company head of Poly Implant Prothese) Jean-Claude Mas’ arrest, people around the world are being reminded of the importance and significance of regulated, quality procedures and materials related specifically to implant surgeries.

Reports everywhere explain how Jean-Claude Mas, who founded and ran the now-defunct implant maker Poly Implant Prothese (PIP), was detained as part of a judicial investigation in the southeastern city of Marseille into manslaughter and involuntary injuries.

“France’s Health Safety Agency has said the suspect implants — just one type of implants made by PIP — appear to be more rupture-prone than other types. Investigators say PIP sought to save money by using industrial silicone, whose potential health risks are not yet clear.
PIP’s website said the company had exported to more than 60 countries and was one of the world’s leading implant makers. The silicone-gel implants in question are not sold in the United States.
According to estimates by national authorities, over 42,000 women in Britain received the implants, more than 30,000 in France, 9,000 in Australia and 4,000 in Italy. Nearly 25,000 of the implants were sold in Brazil.” –Associated Press report

Although [Tamir’s Practice] has never used PIP implants themselves for breast implant procedures, the number of people affected by this discovery makes it highly likely that there are people throughout the US that will be dealing with the ramifications of safe removal and replacement from having the procedure done abroad in the past.

Non-medical grade silicone leaks of any kind can be extremely hazardous to your health and cause serious complications. If you or someone you know has had an implant surgery of any kind outside the US, we recommend that you give us a call and let us help you to determine if you are affected by this news.

This week I was reminded that what may seem like “no big deal,” can indeed become a very big deal. Unfortunately, the reminder came in the form of two patients I saw in the same week with disease that had been neglected over a long period of time.

The first patient was a woman. Her disease had become very advanced and though she claimed to prefer alternative methods of treatment, after a discussion with her naturopathic doctor, it became clear that she was not following any professionally prescribed plan of treatment. As an acupuncturist herself, she had come up with her own idea on how to treat herself, and finally concluded that it was not working. The key to good medicine is the ability to identify when a treatment isn’t working and be willing to accept an alternate form of treatment. By the time she consulted me, the disease had become very difficult to manage.

The second patient was a man who was getting married and was concerned that the skin cancer on his face would look bad in pictures or be hard for other people to see. But, by the time he sat in my office, the disease had progressed to the point that it had taken over half of his face. The surgery required, cost him much more than his vanity.

Both patients, in my opinion, did not have any psychiatric issues or intelligence problems. They’re very functional, attentive in other aspects of their lives, and all-in-all normal people. While their situations were different, it’s interesting how people can allow obvious disease (ulcerations, bleeding, open wounds, clear physical changes with an obvious pathology) to get so advanced.

The moral of this story is one of early detection and to be “better safe than sorry.” Neither one of them woke up one day to their advanced-state of disease. The gradual changes that took place day to day were so gradual in nature that it interfered with their perception of the reality. Words like “cancer” and “disease” are scary, but what can happen when they go unchecked is the real situation to fear. These patients had many reasons for why they waited, but in the end, those reasons did more harm than good.

Patients seek breast reductions for various reasons, ranging from cosmetic to better health and wellness. While we want to improve symptoms there is also a cosmetic element.

Most people initiate the consultation because they have back pain, neck pain, shoulder pain, grooving of the shoulders or, difficulty finding a bra–or sometimes all of the above. A lot of these patients are young patients whose breasts don’t match the rest of their body. Built into every breast reduction is also a breast lift. We don’t just reduce volume but we also reshape the breast to get rid of any droop or sag and replace that with a lifted, perkier breast that will look better in and out of clothing.

The typical surgery will take between an hour and a half and two and a half hours. Although it is an outpatient surgery, we sometimes will keep patients overnight. For some reason (not fully identified), breast surgery patients tend to have nausea. When we keep them overnight, we’re able to monitor them and administer nausea medication with I.V. access, as well as keep them hydrated. This type of surgery is typically not painful, and recovery is relatively short. By two weeks, patients are back to normal activities.

Breast reduction patients are happy patients. We are able to relieve their symptoms and improve their body image. It’s a win-win.

Dr. Tamir Mosharrafa is a board certified plastic surgeon in Phoenix, Arizona.

We are in the very beginning stages of moving our practice. Though the location here in Central Phoenix has served us well, we have outgrown our physical space here.

We have purchased a building at Tatum and Shea which will still provide a nice, central location. All parts of the valley can still get there fairly easily with good freeway access from the 51.

The facility is larger. We will have more space for exam rooms and this will allow us to accommodate more patients. More than one doctor will be able to be in the office at the same time, and will thereby expand our available office time.

The biggest change will be the presence of an in-office operating room. This is another way in which we will be more efficient with our time, have control over the operating room schedule, and increase the availability of operative times for our patients.

We are planning to be moved into the new space after the first of the year and are now working toward planning and developing for that. We look forward to seeing you in our new office!

We are excited to introduce to you a relatively new (to our practice) product: Sculptra. Sculptra has been available for cosmetic use since 2009, and we have been using it here in our practice since 2010.

Sculptra is an inject-able filler. Inject-able fillers have become a very popular method of rejuvenation, particularly facial rejuvenation. Up until now, most have all fallen into one of a few categories; high luronic acid derivatives (Restilin, Juvaderm, Perlene) and calcium based products (Radias and Artfill).

Sculptra is a whole new category. The results thus far, in our experience, have been very good. There has not been a single patient of ours that has been dissatisfied with the results of their Sculptra treatments. It works in a wide range of patients for a variety of applications and is a more direct way to address the signs of aging. The so-called “dermal fillers” inject into the skin and correct lines by way of a sort of camouflage. The problem with the line is not that there has been a loss of tissue in that area, but that other changes have occurred to create the line. In other words, the dermal fillers fill the line, but that is not the primary problem. The primary problem may be a deep loss, bone changes, or the deep fatty layer has been absorbed as one ages. The nice thing about Sculptra is it allows you to put the volume back where it has been lost. They have a famous catch phrase saying that “you didn’t age overnight so why should you anti-age overnight.” This is another distinct advantage of Sculptra because the change is fairly gradual–it happens over a period of weeks rather than instantly. This is an advantage to people who do not want to advertise to the world that they have had “something done.” The results are really impressive, and they last a lot longer. Instead of six months (which is typical for most of the high luronic acid derivatives) it lasts two to three years, at least! We are really excited by this claim and that our results over the past year support this.

If you would like more information about this procedure or other procedures, please contact our office today. We are committed to being your number one resource for cosmetic procedures.
